Williston Basin School District #7 recognizes that parents/guardians have the choice to home educate their child(ren) in accordance to North Dakota Century Code 15.1-23-01 and WBSD#7 Board Policy FDA.
Every year, parents/guardians must file a completed Statement of Intent form (SFN 16909) with the superintendent’s office of their residency district.

Completed Statement of Intent Form (SFN 16909)
A copy of your child’s state certified birth certificate
WBSD#7 requires a copy of the state certified birth certificate for each student, not the hospital copy.
A copy of your child’s current immunization record
Per NDCC 33-06-05, all students must be up-to-date on their immunizations, or claim an exemption by October 1st each school year.
A copy of parent/guardian’s High School Diploma or G.E.D.
Parent/Guardian Current Photo ID/Driver’s License or Tribal Card
Primary Proof of Residence (choose one from items below)
Home Mortgage Statement OR
Notarized Residency Affidavit OR
Builder’s Agreement OR
Purchase Agreement OR
Homeowner’s Insurance Policy OR
Williams County Property Tax Statement OR
Lease/Rental Agreement (must list the names of parents/guardians living the rental unit, along with the manager’s name and phone number)
McKinney-Vento Act:
If you are currently living in a hotel/motel, camper, vehicle, or shared housing with others, you may qualify for the McKinney-Vento Act, a Federal Law designed to remove barriers to education created by homelessness and provide for the educational rights of students in transition.
